Withdrawal/Transfer

A student may withdraw from the school at any time during the school year by observing the following procedures:

  • Inform the Registrar’s Office of the intent to withdraw from the school.
  • Secure clearance form from the same office.
  • Have this from signed by the Guidance Office Staff, Library staff, Class Adviser, Principal, Accounting Office personnel and the Registrar’s Office personnel.
  • Request the report card (F-138) from the class adviser.
  • Have the report card signed by the registrar for transfer.
  • Ask for the health and guidance records from the clinic and Guidance Center, respectively, if transferring abroad.
  • Settle the following at the Accounting Office:

o Back accounts, if any
o Payment for official transcript of records (F-137)
o Other financial obligation with the school

No accounting clearance will be obtained unless all the above obligation are paid in full.

  •  Present receipt of payment at the Registrar’s Office, which will, upon request, forward the F-137 to the next school.


A student will be issued the final F-138/Report Card with the accompanying Certificate of Eligibility for Admission and/or final F-137/Transcript of Records, whether for evaluation or transfer purposes, only after settling financial or property obligation with the school. The same policy applies for issuance of duplicate copies of either F-137 or F-138.
