Please be informed that the DVA will begin accepting applications for the 2026 – 2027 DVA-CSA stickers on Monday, May 11, 2026.
Procedure:
1. Read the DVA-CSA Vehicle stickers Issuance Guidelines and the DVA Traffic Rules and Regulations by clicking on the following link: https://dva.org.ph/rules-security-csa-vehicle-stickers…/
2. Enter the following on your web browser: https://form.jotform.com/dvai/csastickers to start your application process. Please note that the link will only be active beginning May 11, 2026.
3. Documentary Requirements:
β€’ Vehicle Certificate of Registration (CR) and latest Official Receipt (OR)
β€’ For vehicles not registered under the name of the applicant, bring a certificate from the vehicle owner designating the vehicle to the sticker applicant.
Additional requirements:
β€’ For parent(s), legal guardian(s): CSA Official Receipt of Student Registration/Enrollment
β€’ For CSA Teachers/Staff: Valid CSA Teacher/Staff ID
β€’ For School Service Operators: Valid Accreditation from CSA.
