Who can request a Certificate of Enrollment for a student?
The certificate of enrollment may be requested online or onsite. To request online, send an email to registrar@csa.edu.ph containing the complete name of the student, the name and relationship of the requesting party, and the purpose of the request. To request onsite (or in-person), the requesting party will be asked to fill out a form. Turnaround time for an acknowledged request for a certificate is 3-5 working days.
How can I apply for ESC or SHS VP?
ESC and SHS VP are grant programs of PEAC or the Private Education Assistance Committee and the Department of Education or the DepEd. For ESC, application is open only for incoming grade 7 students. Requirements include: Letter of Intent addressed to HS Principal, Fr. Julian C. Mazana, OSA, copy of final report card, 1x1 ID picture, Proof of income of parents, and accomplished ESC application form For SHS VP, application is through the PEAC OVAP website. Unlike ESC, SHS VP can only be applied by incoming grade 11 students or their parents. Please visit the website for details of deadlines and requirements. Successful applicants for SHS VP will be issued a QVA which will have to be submitted to the Registrar’s Office on or before July 30, 2025.

I lost my diploma/report card. Can I request for a new copy?
Duplicate copy of lost school records such as diploma or report card may be issued upon presentation of affidavit of loss. Damaged records may be replaced with duplicate copy upon submission of original copies. Turnaround time - 7-10 working days
How do I report the correction of my name?
Parents may file a correction of the name of their children upon submission of the original PSA birth certificate. Depending on the case, the Registrar may also request for other legal supporting documents.
Where can I find my Learner Reference Number (LRN)?
The Learner Reference Number may be found on the upper left hand corner of the Report Card.
Can my school records be sent online?
Not at the moment. We are still finding a more secure way of sending school records online.
